Welcome to Extended Family Associates

We help families keep their loved ones at home, helping them 'age in place,' allowing sons and daughters, as well as other relatives, to step out of the role of caregivers and back into the role of family and friends. I will use my experience as a case manager in-home care, hospice care, and nursing homes to give guidance and support to immediate and extended families.

As an Aging Life Care Manager, I know that most individuals want to stay in their own homes. We coordinate/provide a variety of services allowing individuals to remain independent at home. We provide Professional Health Care which we coordinate with the Primary Physician, such: as medication management, consulting with the client's pharmacy, checking drug interactions.

We assist in navigating the Health Care System, accompanying clients to physician appointments, knowing the right questions to ask, and advocating for our clients with attending and consulting physicians. We develop a Plan of Care with the clients; their goals become our goals, we share the plan with attending physicians as necessary.

Our plan is to honor the client's and family's wishes, our visits are intermittent, and the frequency of those visits is guided by the client's/family's desires, allowing your loved one to stay in their homes and remain independent for as long as possible. Because of our experience, we can bring in the members of a team to help our loved ones stay at home.

We can coordinate personal care and hygiene, help obtain Durable Medical Equipment, support long distant families, coordinate meal planning and shopping, consult with Physical Therapists, Occupational Therapists, and Speech Therapists in the home or an outpatient. This can be more cost-effective than placement in assisted living facilities or nursing homes.
